When the form has been completed, call 250.740.6446 to make a one-hour appointment with a Accessibility Services Education Counsellor to discuss your accommodation supports for school.
- Verification of Disability – This form is needed for students who do not have medical information to document their permanent disability. Your Dr. or medical specialist completes Section 4.
- Temporary Disability – Use this form if your disability is short term and not permanent.
